Root Cause Analysis Made Simple with Susan Lubell

This episode of the rooted in reliability podcast covers the Root Cause Analysis in the light of a book written by Susan Lubell – the guest herself—and the book is part of the made simple series. In this book, Susan has covered a number of matrices that need to be answered very specifically and clearly based on the methodologies adopted by different organizations. This book doesn’t just focus on the terminologies and specific techniques that are used by the experts. It actually covers the very baselines of the RCA methods such as how to get started with the analysis and what to look for in order to successfully implement it. This book is more of a guide for those who want to start their journey in the field and can use a bunch of best practices that are used by the top-notch organizations in the industry. It’s not about the fault tree analysis or other techniques that are used to determine the root cause. It’s more about the people managing side of the Root Cause Analysis like why do you need to perform an RCA and what are the factors that drive it.

The main goal here is to make sure that failures don’t occur again ever or even if they do somehow, their consequences are minimized. For that purpose, you need to figure out who to involve, how to get the job done, and how the processes should be carried out. Basically, you need to come up with a simpler model that you have to follow in order to prevent the damage to your assets and then you can improve that earlier planned model with time. So you have to identify the reason for failure to your equipment or make the most out of business opportunities that are presenting themselves to your organization.

You not only have to implement all the corrective actions that are included in your plan but also you need to see if those actions are truly preventing the failures and bringing value to your organizations. You also need to choose your team very carefully because they are ones who will come up with the solutions to figure out what actually caused the production to stop in the first place and how to get it back online quickly by implementing those quick actions that are within the budget and time restraints of the organization.

You don’t have to start solving the complex problems immediately. You can start with simpler failures in the beginning and once you have that experience, it won’t be a problem to understand the complex failure mechanism and that what RCA Made Simple by Susan Lubell and Ricky Smith is about. It starts with simple why, when, who and how with step by step approach in each chapter—six chapters in total—and then goes to identifying the smaller problems and listing and evaluating all the possible solutions. In the end, you have to document everything and implement all the corrective actions.
