What is an FMEA and How Can It Help?

Failure modes and effects analysis (FMEA) is a step-by-step approach for identifying all possible failures in a design, a manufacturing or assembly process.

“Failure modes” means the ways, or modes, in which something might fail.

Failures are any errors or defects, especially ones that affect the customer and can be potential or actual.  They typically include the component, the damage or problem with it and the cause of it

“Effects analysis” refers to studying the consequences of those failures.

Failures are prioritized according to how serious their consequences are, how frequently they occur and how easily they can be detected.

The purpose of the FMEA is to take actions to eliminate or reduce failures, starting with the highest-priority ones.

By identifying all of the failure modes and the consequences of each, a preventative or corrective action can be selected to improve plant performance.

    Failure Modes, Mechanisms and Effects Analysis (FMMEA) enhances the value of FMEA and FMECA by identifying high priority failure mechanisms and failure models. High priority failure mechanisms determine the operational stresses, and the environmental and operational parameters that need to be controlled. Models for the failure mechanisms help in the design and development of a reliable product.

    CALCE ( http://www.calce.umd.edu/ ) has developed the FMMEA process and helped several organizations implement the tool in their product development process

      Failure mechanisms are vitally important to understand when developing PM, CBM, or PdM activities. If you do not understand the mechanism of the failure, most maintenance activities will not be effective in preventing the failure mode.

      I have not seen the tool you have linked to, but will be looking into it. Aside from that tool, what other resources do you have for failure mechanisms?
